Note: Awake DBT is an out-of-network provider. If you have a PPO plan, your insurance company will reimburse at a level deemed ‘reasonable and customary.’ We encourage you to contact your insurance provider to determine how much they will reimburse for individual DBT therapy (procedure code CPT# 90834) and skills group (procedure code CPT# 90853), so that you will be aware of your out-of-pocket expenditures.
If needed, we can negotiate a single case agreement with your insurance provider, which is essentially a customized contract with Awake DBT. We will negotiate with your carrier to provide DBT treatment for a set fee. We will ask for your help to initiate the process as it can take several weeks to establish the agreement.

If your insurance has denied your request for a Single Case Agreement, or what was reimbursed was not the amount quoted to you by your insurance company, or if you have a complaint to file about the process, fill out and send the attached Independent Medical Review Application/Complaint Form to the California Department of Managed Health Care to review your situation.
